Assistant Practitioner Carlisle & District ICC

An exciting opportunity has arisen at the Carlisle & District ICC for an enthusiastic Qualified Band 4 Assistant Practitioner to join our dedicated team. A great opportunity to try out community ICC working.

This varied role will involve working with community patients, working on their rehabilitation goals, as well as completing rapid response visits with an emphasis on supporting discharge and admission avoidance.

The candidate would need hold the band 4 Assistant Practitioner qualification. Trainee APs would not be considered at this time.

As an assistant practitioner, you will be part of a multidisciplinary team including occupational therapists and physiotherapists working alongside other health and social care professionals to develop integrated services ensuring that patient care is delivered to the highest possible standard.

The post holder will implement care for patients in the community and other settings, observe the patients condition, provide interventions with a rehabilitation therapy focus and evaluate outcomes.

Flexible working, five days per week, including weekends and bank holidays will be required to meet the needs of the service.


Main duties of the job

* Manage a community therapy caseload, completing rehabilitation treatment plans and treatment sessions.
* Work closely with Occupational Therapists, Physiotherapists, District Nurses and wider ICC team members on joint treatments and visits.
* Support admission prevention through rapid response visits.
* Follow the D2A model in both community hospital and acute settings to support discharges.
* Complete a variety of assessments including personal care, kitchen assessments, mobility assessments, falls and environmental assessments.
* Assess and provide equipment and adaptations.
* Work with patients to maximise their rehabilitation potential which includes providing actioning therapeutic activities.


Qualifications

* Have a Foundation Degree - Health and Social care, (Assistant Practitioner route) or equivalent. Maintenance of professional portfolio.
* Level 2 Maths, Level 2 English or equivalent, Care Certificate.


Knowledge and Experience

* Significant experience of working within a healthcare environment.
* Experience of caring directly for patients, relatives and carers.
* Experience working in a multidisciplinary team.
* A basic understanding of the use of IT.
* Understanding of and adherence to boundaries of practice and professional conduct.
* Experience of working in a community rehabilitation setting.
* Experience of working in health promotion.
* Experience of using outcome measures.
* Experience of delivering group intervention.
* Knowledge of community working.
* Knowledge of exercise and activity recommendations for healthy lifestyles.


Skills and Aptitudes

* Excellent interpersonal and communication skills.
* Ability and commitment to complete studies at higher education level including self‑directed learning.
* Ability to independently use email and computer based systems.Ability to work within the team and independently.
* Ability to use own initiative.
* Ability to organise, prioritise and re‑prioritise workloads.
* Ability to demonstrate the compassionate values and behaviours needed for dignified care.
* Motivational skills.
* Developed observational skills.


Other Requirements

* Willingness to further develop the role.
* Ability to travel independently of public transport, Full Driver’s Licence.


Disclosures and Requirements

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and to a Disclosure and Barring Service check. The cost of the DBS is met by the successful candidate through salary deduction, as follows:

* Standard Check: £21.50 + £6.50 administration = £28.00, deducted from salary over 4 months or one‑off payment.
* Enhanced Check: £49.50 + £6.50 administration = £56.00, deducted from salary over 4 months or one‑off payment.
