Main duties of the job
The successful candidate will:
* Play a key role in the delivery of generaltreatment room duties includingvenepuncture, ECGs immunisations and wound care.
* Chronic disease management, in particulardiabetes
* Work with the wider MDT to ensure safe andeffective patient centred care.
* Participate in the development of protocols,audits and service evaluation.
* Engage in professional development and mandatory
* training.
* Support the self-management of patients through education, advice andhealth promotion.
* Manage workload effectively, prioritisingpatient needs while maintaining high standards of care andprofessionalism.
* Ensureaccurate clinical documentation in line with NMC standards.
Willingness to travel between sites whenrequired

Job responsibilities
Main Responsibilities :
To assess the needs of patients, providingappropriate care/treatment in conjunction with the GP, or independently, asappropriate.
Refer to colleagues or other agencies asappropriate.
Provide the full range of practice nursing services,including venepuncture, ECG measurement, blood pressure measurement,urinalysis, vaccinations, swabs, health checks, assessment of risk factors,lifestyle advice, weight management, smoking cessation, ear care, sutureremoval, wound care, family planning, etc.
To manage the care of patients with long termconditions. This may include organisational responsibility for specific diseaseareas.
Assess, plan, implement and evaluate treatmentplans for patients either with or at risk of developing a long term condition which meets identified needs and promote health and well-being.
Select appropriate tests for patients, andinterpret and act on results where appropriate.
Support effective treatment and prescribing byworking with patients in order to support adherence to prescribed treatments,reviewing medication for therapeutic effectiveness, and providing informationand advice (including over-the-counter medication) on medication regimens,side-effects and interactions.
Educate and support patients as needed, includingadopting healthy lifestyles, appropriate self-care for minor illness andfollowing care plans for long term conditions.
Provide a sexual health service where appropriate.
Provide a minor illness service.
Prioritise health problems and interveneappropriately to assist the patient in complex, urgent or emergency situations,including initiation of effective emergency care.
Organise and/or run initiatives, includingcontacting patients, to improve uptake of screening, vaccinations or otherservices and support the achievement of targets (e.g. under the Quality andOutcomes Framework and other commissioned services).
To maintain accurate and contemporaneous records torelevant professional standards/guidelines.
Other Responsibilities:
Assist the partners and lead nurse in planning anddeveloping new nursing services as required.
Assist whenrequired the lead nurse ensuring nursingguidelines, policies and other documents (including PGDs) are up-to date
Assess, advise on, implement, monitor and ensure thequality of enhanced services as requested.
Participate and support with the implementation ofinduction programmes for new nursing staff.
Assist in assessing competence of clinical staffwhere required, including in-house certification of staff where appropriate(e.g. Healthcare Assistants).
Actively promote the workplace as a learningenvironment, encouraging everyone to learn from each other and from externalgood practice.
Take responsibility for own development, learningand performance including participating in clinical supervision and training asnecessary to update skills and knowledge.
Participation in the educational and trainingevents and meetings of the Practice.
Participation in the induction of new staff,students and other attached professionals as necessary.
Support the nursing team by providing routine and unplanned cover across all sites when required.

Qualifications
* Registered General Nurse
* Current NMC registration
* Level 6 in Diabetes management or above
* Proficiency in using EMIS
Experience
* Experience of nurse led chronic disease management clinics, in particular Diabetes
* Experience in venepuncture, immunisations and vaccination administration.
* Performing ECGs.
* Proficient in lower limb assessment and management, including vascular assessment and compression therapy.
* Proficient in assessment and management of acute and chronic wounds.
* Experience in long term conditions, particularly Diabetes or willingness to undertake training.
* Upto date training and knowledge in childhood immunisations or willingness to undertake training.
* Managing and ordering stock where necessary
* Experience working autonomously within their scope of practice.
* Excellent communication and interpersonal skills.
* Have a clear understanding and compliance with all infection control principles
* Have a clear understanding of adult and child safeguarding.
* Experience in a Primary Care Setting
* Cervical Screening.
* Experience in assisting with minor operations clinics.
* Up to date knowledge and training in travel health.
