The Cardiology ANPs at Glan Clwyd Hospital provide a high-quality, comprehensive and innovative nurse-led service for patients experiencing Acute Coronary Syndromes. They provide and facilitate timely care and interventions for patients with Acute ST Elevation Myocardial Infarction (STEMI) or high-risk Non-ST Elevation Myocardial Infarction (NSTEMI) for North Wales and complete care with "Post Intervention Clinics".
The team consists of six ANPs (full-time and part-time) who work closely with all members of the Cardiology multi-disciplinary team. They provide inpatient care for patients presenting with an acute cardiology condition and outpatient care for those attending the Rapid Access Chest Pain Clinic. They offer expert advice and support to other clinical colleagues, particularly Acute Medical Consultants and Cardiologists, and provide education, training and clinical expertise to primary and secondary care staff.
The post holder will practice under supervision as a trainee advanced practitioner within cardiology, encompassing assessment, examination, diagnosis and treatment within an agreed scope of practice throughout cardiology services.
Main Duties of the Job
* Work alongside the ANPs coordinating Primary PCI calls.
* Identify patients with acute coronary syndrome within the emergency quarter.
* Assist and advise allied health professionals MDT on ECGs and treatment of STEMI and ACS.
* Work closely with CCU and MDT to ensure cardiac patients receive quality care in the correct environment.
* Complete local and national audits.
* Teach junior members of staff and other members of MDT.
* Have the ability to speak Welsh (desirable); Welsh and/or English speakers are equally welcome.
* Learn to identify critically ill cardiac patients within the acute setting, including those who require PPCI.
* Escort patients throughout their PPCI pathway acting as support to the Catheter Laboratory Team.
* Develop confidence and competence to autonomously run RACPC with responsibility for clinical standards, competence assessment and supervision of other health professionals.
* Advising in the management of ACS patients in all areas of the hospital including unscheduled care.
* Collaborate with primary care colleagues to understand specialist cardiology teams across primary and secondary care.
* Develop a knowledge base to deliver specialist education and training to a broad range of health professionals including nurses, doctors and ambulance personnel.
* Undertake research and lead clinical audit in own speciality with organisational responsibility for compliance with relevant national audit programmes.
* Complete clinical competencies to support the Cardiac Catheter Laboratory in the North Wales Primary Percutaneous Cardiac Intervention Service (PPCI).
* Recognise patients with changing health status and take appropriate actions.
* Communicate highly complex and highly sensitive information with patients, families and within the healthcare team.
* Ensure safe use of highly complex patient monitoring and support equipment within the ED/CCU/Catheter Lab.
* Interpret laboratory and radiological results and initiate appropriate patient management.
* Maintain personal clinical development by keeping abreast of new treatments and technologies within cardiology.
* Review with a supervisor all aspects of the patient’s plan of care and identify and address issues that may result in a sub-standard service.
* Safely triage referral of patients with suspected Acute Coronary Syndrome (ACS) and conduct comprehensive clinical assessment.
* Transfer patients for primary PCI to the Catheter lab and to CCU/ward if necessary.
* Evaluate care provided, order necessary investigations, and take responsibility for acting upon results, providing treatment and care individually and through referral to cardiologists or specialists as appropriate.
* Provide expert professional advice and liaise with patients, the Emergency Department, Acute Admissions Unit, Cardiac Catheter Laboratory, Coronary Care Unit and Cardiology Ward and colleagues.
Person Specification
Essential Criteria:
* Registered Nurse
* Extensive cardiology experience
* ILS or ALS
* Evidence of advanced ECG interpretation
Desirable Criteria:
* ALS or willingness to undertake
* CCU/Cath lab competencies or willingness to undertake
Personal Qualities
Essential Criteria:
* Highly developed communication skills to be applied at all levels within situations requiring persuasive, motivational, negotiation, empathetic or reassurance skills.
Desirable Criteria:
* Evidence of involvement in leadership, team building, managing resources, time management, change management / clinical developments, audit and research.
