Job Overview
This role is open exclusively to current NHS staff in Cornwall or the Isles of Scilly (Cornwall Foundation Trust, Royal Cornwall Hospitals NHS Trust, Cornwall and Isles of Scilly Integrated Care Board). You will work within the Cornwall Onward Care Team at University Plymouth Hospital, Derriford, supporting the smooth discharge of patients.
Main Duties
* Ensure patient needs are addressed timely and in accordance with care plans.
* Sit as the link between agencies and departments to expedite onward care.
* Develop relationships with ward teams to coordinate flow across the acute trust.
* Support the Integrated Transfer of Care Hub in daily flow management.
* Contribute to follow‑up assessments, care planning, resource identification and service delivery to facilitate prompt patient discharge.
Person Specification
Education / Qualification
* NVQ Level 4 qualification in a relevant subject area or equivalent experience.
Essential Criteria
* Demonstrable knowledge of patient flow.
* Ability to create innovative solutions to support discharges.
* Ability to act calmly in emergencies and respond professionally to stressful situations.
* Ability to prioritise workload for self and others.
Desirable Criteria
* Experience supervising or managing people.
Skills and Aptitude
* Calmness and professionalism under pressure.
* Effective prioritisation of workload.
Knowledge and Abilities
* Strong knowledge of patient flow processes.
* Innovative problem‑solving skills to enhance discharge procedures.
