The East London Intensive Support Team provides a 7 day a week urgent and intensive community response for people with Learning Disabilities with additional complex mental health needs and/or behaviours that services perceive to be challenging, that result in the person posing a risk to themselves or others. The service operates across the inner east London boroughs of Tower Hamlets, Newham and City and Hackney.
The post holder will assist other professionals in the IST in providing assessment, care, treatment and discharge planning. This will be achieved through close collaboration with the person, carers, colleagues and other service providers. This may include supporting people during admissions to acute mental health wards.
The post holder will work directly with service users to develop a rapport based upon respect, kindness and compassion that builds the basis of a professional relationship. You will have a significant role in supporting service users to engage with their agreed Care Plans, positively promoting independent living and recovery.
You will work as a part of a multidisciplinary team, building relationships within the Intensive Support Team as well as with the Community Learning Disability and Inpatient services and Local Authorities across east London.
Main duties of the job
* To support the Intensive Support team in providing assessments, care and treatment for People with learning Disability, Mental health issues and behaviours which challenge in the community under direct or indirect supervision of the team.
* To use skills in working with people with learning disabilities who are experiencing a Crisis to maintain, promote and improve health.
* To provide advice and consultation to colleagues supporting Autistic Adults who do not have Learning Disabilities.
* To facilitate access to health services through collaboration and partnership with primary and secondary health, and other relevant services.
* Developing working relationships with clients, provider organisations, GPs, local community resources.
* Support in the delivery of generic health tasks such as the completion of Hospital Passports, Health Action Plans, monitoring of mental health and behaviours which may challenge.
* Prepare clients and carers for health appointments to enhance their understanding to enable them to actively participate in their own health care.
* To contribute to shaping local health services in the way they are organised and run to promote an inclusive service for people with learning disabilities.

* To support the Intensive support team with allocated caseload and support the team in meeting the needs under the direct or indirect supervision from the nursing staff
* To work with multi professional teams across the three boroughs of Tower Hamlets, Newham and City & Hackney providing a positive role model to support needs of servicer user in relation to their mental health and or behaviour that challenges.
* To use a person-centred approach to promote choice, independence and inclusion.

Clinical
* To establish a therapeutic relationship and communicate effectively with service users, family carers and paid carers to implementintervention plans, with sufficient explanation to ensure informed consent
* To be able to adapt verbal and written communication and interaction style to meet the needs of the client and carers to ensure most effective outcome from intervention
* To work through interpreters with service users/family carers where English is not the first language in order to ensure effective communication and sensitivity to religious and cultural needs
* To work with service users to gather feedback and ideas for the development of the service
* To communicate effectively with those across client networks, for example, Community Learning Disability Services, staff working for provider organisations in day services and supported living placements.
* To co-facilitate the delivery of group educational sessions for participants, family members and keyworkers
* To support participants to develop/update Health Action Plans, Hospital Passports and Positive Behaviour Support plans
* To be aware of risks involved in providing care to service users and their families and to ensure self and client safety.
* To work as part of a multi-disciplinary team providing care to service users within the community under the direct or indirect supervision of Intensive Support team
* To participate in the assessment, planning, implementation and care plan under the supervision of qualified staff
* To participate in the regular reviews and discussions of the Intensive support team caseload.
* To initiate, organise and evaluate therapeutic support with clinical advice as appropriate.
* To be able to utilize skills flexibly so that interventions can be creatively thought about and provided at clients homes, schools, clinics or other appropriate settings
* To be able to recognise child and adult in need and protection situations and be aware of and adhere to Safeguarding Guidelines and know when to inform and share these concerns with other colleagues
* To assist in the implementation of programmes of care as requested by the Intensive support team
* To ensure that clients receives a culturally competent service.
* To identify and provide the necessary support to service users to help them meet their health objectives.
* To observe and report any changes in the service users physical, mental health and behaviour to Intensive Support team
* To be aware of community resources and facilitate the appropriate referral of service users to those best able to help them.
* To maintain accurate and timely information, documentation and reporting systems in accordance with agreed standards, Trust Policies and the Data Protection Act.
* To participate in audits and survey relevant to the team.
* To have an awareness and understanding of effective risk management techniques.
* To have an awareness of service standards/targets and ensure they are being met.
* To receive regular feedback from service users and carers on the service provided by the team and ensure the Intensive Support Team Manager is aware of any areas of concern, including the completion of routine outcome measures.

Administration

Management
To receive regular clinical and management supervision
Adhere to established policies, procedures, protocols within the service and those specifically relating to the A&E Liaison Crisis Team.
Appropriately and without undue delay seek support and consultation regarding cases.
Liaise with and develop positive partnerships and relationships with local stakeholders.
To maintain an awareness of current developments in the field of child and adolescent mental health / Crisis services by attending training courses and workshops and having access to current literature.
Participate in regular shared learning activities and developmental experiences in the Intensive Support Team
Human Resources
* To liaise and develop working links with local community organisations and providers and attend meetings as required by the post

To attend regular supervision and obtain supervision/support from other members of the team, as appropriate

Person Specification
Education/ Qualification/ Training
* NVQ Level 3 qualification to diploma equivalent level or above
Experience
* Experience of working with people who have a disability and/or physical/mental health/sensory difficulties
* Experience of using Excel software for data recording and reporting
* Experience of team working
* Experience of co-facilitating and presenting information to groups
* Experience of working with people who have a learning disability, mental health issues and behaviours which may challenge
* Experience of working with people who have communication difficulties or are non-verbal
Knowledge and Skills
* Understanding of what a learning disability is and the impact it may have upon someone on a day to day basis
* Ability to adapt communication style to communicate effectively with those who have limited verbal communication skills
* Good interpersonal and written communication skills
* Competence in the use of IT software - Microsoft Word, Excel, PowerPoint, Outlook, Teams; Zoom
