Overview
We are pleased to invite applications for the position of Practice Nurse at a respected and forward-thinking GP surgery in Portsmouth. This opportunity is open to experienced nurses seeking either a permanent post or a long-term locum assignment, depending on preference and availability. The practice is committed to delivering high quality, patient-centred care and is supported by a cohesive clinical and administrative team.
Responsibilities
* Manage patients with chronic conditions such as asthma, diabetes, and hypertension
* Undertake cervical screening, immunisations, wound care, and travel health advice
* Support health promotion and patient education programmes
* Assist in the achievement of QOF and other clinical targets
* Collaborate effectively with GPs, healthcare assistants, and administrative staff to ensure continuity of care
Requirements
* Registered Nurse with valid NMC registration
* Demonstrable experience in a primary care or GP surgery environment
* Proficiency in treatment room duties and chronic disease management
* Knowledge of clinical systems such as EMIS or SystmOne (preferred but not essential)
* Strong communication, organisational, and interpersonal skills
* Ability to work independently as well as part of a multidisciplinary team
