Your Position
* Assist patients after hospital discharge.
* Support clients in regaining independence and confidence.
* Deliver care in line with individualised care plans.
* Provide:
o Medication administration
o Monitoring vital signs
o Exercise support and rehabilitation facilitation
o Household chores
* Communicate with medical staff, including district nurses and pharmacists.
* Work independently and collaboratively as part of a care team.
* Maintain accurate electronic reporting and documentation.
Hours of Work
Full‑time contract up to 40 hours per week, with shift patterns ranging from 7:00 am to 8:30 pm. Flexibility to work different shifts is required to meet service needs.
