Rate of Pay : £14.09 - £18.42 per hour Hours per week : 18.50 Summary: The Reablement Care Worker joins the Community Resource Team (CRT) Homecare to support citizens throughout Cardiff in living independently within their own homes. The primary focus of this role is to prevent unnecessary hospital admissions and facilitate timely discharges by assisting service users in achieving their specific reablement and wellbeing goals. Working in a dynamic, fast-changing environment, you will collaborate closely with health professionals to deliver person-centered care based on individual assessments and service delivery plans. Responsibilities: Support service users to achieve their individual reablement goals and what matters most to them. Follow individual service delivery plans to assist carers in their daily duties and ensure individual needs are met. Maintain essential observation and recording skills to track progress and changes in a service user's condition. Actively support the safety and protection of vulnerable adults in line with the Councils safeguarding priorities. Qualifications: A caring nature and the ability to undergo training to achieve the relevant Health and Social Care (HSc) level required for the post Requirement to register with Social Care Wales Essentials : Female candidates are preferred A full UK driving license Please note: You should be available to work immediately or at a short notice.
