Locum Consultant in Community Care of the Elderly/Frailty
An exciting opportunity has arisen for a Locum Consultant Geriatrician to join the Community Care of the Elderly and Frailty teams in Chepstow Community Hospital. The successful candidate will provide senior clinical leadership to the inpatient community medicine ward and the Monmouthshire Rapid Frailty Response service, supporting timely decision-making for the older and/or frail adult.
The post holder will play a central clinical leadership role in promoting multi-disciplinary working, the use of evidence-based practice and ensuring that the principles of clinical governance are fundamental to the design, delivery and evaluation of new and existing models of care.
This is a hybrid role that centres on early intervention and proactive management of older and/or frail individuals enabling care to be provided in the most appropriate setting at the right time.
Main duties of the job
* To support the inpatient medical ward as named physician responsible for patient care
* To perform a key role in discharge planning and co-ordinating approaches with relatives and other agencies
* To liaise with patients, relatives/carers and members of the multi-disciplinary team to ensure patients receive a high-quality service and are kept informed of progress as appropriate
* To lead ward rounds, facilitating multidisciplinary meetings
* To manage referrals into the North Monmouthshire/Frailty service, undertaking clinical assessment and treatment planning to ensure patients receive care in the most appropriate setting
* To support the delivery of care on the ward and within patients' homes, as appropriate, providing clinical intervention where required
* To signpost and redirect patients to appropriate community services, ensuring care is co-ordinated and aligned with individual needs
* To support early supported discharge to ensure safe and co-ordinated transitions from hospital to services within the community
* To contribute to the design, evaluation and improvement of Frailty pathways, ensuring they align with best practice and national guidance
* To work collaboratively with senior clinical colleagues, multidisciplinary teams and partners to enhance the effectiveness and reach of Frailty services
* To participate in quality improvement programmes and other initiatives promoting safe, efficient and patient-centred care
