What You Will Do:
* Care Planning & Delivery: Assess, plan, implement, and evaluate individual care plans, ensuring that high standards of evidence-based care are consistently maintained.
* Autonomous Practice: Work independently, recognising changes in patients’ presentations that require intervention, escalating concerns to senior staff as necessary.
* Leadership & Mentoring: Guide and direct junior staff in delivering care programmes, ensuring the welfare of patients and supporting their personal development.
* Patient Support: Assist patients in daily living activities, acting as a health promoter and educator for both patients and their support networks.
* Communication: Communicate effectively with patients and their families, respecting their views and cognitive abilities, while maintaining confidentiality at all times.
