To work in the community with individuals aged 65 and over who would benefit from short-term low-level support to help them remain independent in their own homes and reduce reliance on Adult Services and NHS services. This is a 1-year initial contract with the possibility of extension subject to funding.
Responsibilities:
1. Enable independence and provide encouragement to avoid dependency or worsening circumstances.
2. Promote individuality, choice, dignity, respect, and partnership working.
3. Help individuals identify personal goals.
4. Safeguard vulnerable people from abuse and report any suspected abuse.
5. Provide person-centred Home Support, including:
1. Connecting clients with social support schemes and community resources.
2. Facilitating 'Warm handovers' to organizations.
3. Reducing food and fuel poverty risk.
4. Assisting with assistive technology and home environment improvements.
5. Supporting financial management and benefits applications.
6. Making calls and assisting clients with appointments.
7. Building community relationships and working with other agencies.
8. Supporting clients with hoarding behaviors with empathy and patience.
9. Maintaining confidentiality and accurate records.
10. Participating in projects, producing reports, and attending meetings.
11. Providing holiday cover and undertaking relevant training.
12. Adhering to quality standards and policies.
6. Experience in Community Development, Health & Social Care, or equivalent.
7. ICT skills, including Microsoft Office, email, and internet.
8. Experience working with vulnerable people and problem-solving skills.
9. Ability to meet deadlines, monitor services, and produce reports.
10. Strong interpersonal skills and ability to work independently and in teams.
11. Knowledge of the Voluntary Sector.
12. Availability for additional hours and unsocial hours as needed.
