Advanced Pharmacist
Join to apply for the Advanced Pharmacist role at Hywel Dda University Health Board. This position is part of the multidisciplinary inflammatory bowel disease (IBD) team and offers a split posting across several candidates with both full and part‑time options.
As an Advanced Pharmacist you will work alongside consultants and specialist IBD nurses to optimise treatments for IBD patients, run pharmacist‑led clinics (including virtual clinics), and support the development of non‑medical prescribers if required.
Responsibilities
* Develop clinical skills to assess disease activity and formulate management plans.
* Review patients on immunomodulatory and biological drugs to ensure optimal treatment and appropriate monitoring.
* Lead on biosimilar switches and dose optimisation.
* Monitor financial drug expenditure for the specialty and lead cost and service improvement programmes.
* Collaborate with the multidisciplinary team, homecare team, and external providers.
* Create and maintain up‑to‑date prescribing guidelines and monitor their implementation.
* Provide specialist pharmacology services to meet high standards of patient care and safety.
Qualifications & Experience
* Qualified pharmacist with experience in IBD or a related speciality.
* Experience in managing complex medication regimes, including biologics and biosimilars.
* Strong communication, self‑motivation, and innovative problem‑solving skills.
* Desirable: non‑medical prescriber status or willingness to undertake qualification.
* Desirable: ability to speak Welsh; English proficiency is mandatory.
