Social Worker - Learning Disability Team x 2

To deliver a high quality,strengths-based approach to statutory social care service, ensuring that adultsand their families and carers

* Areable to seek strengths-based solutions, assess, manage and review their ownneeds and risks, maximise their own independence and self-direct theirresponses to deliver agreed outcomes
* Canaccess personalised care and support to meet their assessed needs and maintainthemselves in the community

Main duties of the job

Ensure that people are fully involved and empowered throughout their contact with the service.

Ensure that the person is fully understood in order that the most appropriate tailored solutions can be offered.

Identify and address any system barriers which prevent the people from experiencing the most efficient and effective journey through the service.

Ensure that people are empowered, connected, independent and have an improved sense of well-being through the provision of outcome focused support

Meet with people regularly to discuss their experiences of the service to drive further improvement.

Job responsibilities

Performance

* Workto the teams operating purpose and principles to help individuals live thebest lives they can, whilst working within the relevant processes, policies andprocedures.
* Fulfilthe full range of the services statutory responsibilities with regard toindividuals and carers who present with a need for care and support.
* Assistwith the allocation of referrals to the appropriate team member based oncomplexity of need, skills set required and worker capacity; escalate to seniorsocial worker/team manager for action if capacity is not available as required.
* Ensurethat individuals are protected from abuse and neglect by undertakinginvestigations into Adult Safeguarding enquiries and completing written andverbal reports outlining findings/ conclusions.
* Whereidentified as the lead worker, be responsible for ensuring that the servicemeets what matters to each citizen.
* Undertakework with individuals in situations of high risk or complexity, utilising arange of evidence based social work models and approaches.
* Alwaysseek to optimise the participation of citizens in the assessment and, planningof support utilising the Mental Capacity Act where appropriate to support andunderpin decision-making.
* Helpto develop the strengths and assets of individuals and their networks.
* Act toeffectively prevent, reduce or delay the need for care and support prior tolonger term support services being put in place.
* Ensurethat the individual and/or their representative is fully aware of the financialimplications of their care and support arrangements, arranging timely FinancialAssessments and inputting service package details onto Care First.
* Maintainelectronic and professional records to demonstrate professional accountabilityand support performance reporting.
* Provideguidance and support to unqualified colleagues, as required and within theremit of your role and experience.

* Utilisethe full range of council processes, policies and procedures that relate to theeffective, safe and accountable operation of the business.
* Contributeto the use of data to ensure that team resources are used effectively andefficiently.
* Utiliseevidence relating to personal and team performance to identify and addressareas for development.

* Obtainfeedback from citizens on the effectiveness and quality of our service.
* Contributeto the development of local networks and community resources by developing andmaintaining effective partnerships with a range of professional, Third sectorand citizen partners.

Person Specification


Qualifications

* Have a recognised Social Work Qualification
* Be registered or in the process of registering with the HCPC
* Practice in accordance with HCPC standards at all times


Knowledge, Skills and Abilities

* Demonstrate a high level of organisational skills; has Order as a strength being able to organise and plan activity with the team and coordinate complex actions
* Demonstrate excellent Judgement; be able to make the right decisions quickly and easily even when faced with pressurised situations
* Demonstrate effective skills in Persuasion; is able to bring others round to a new way of thinking as a way of helping them to change. Is confident in positively challenging colleagues and agencies in order to achieve the best outcome for citizens.
* Possess excellent verbal, written communication and analytical skills
* Have excellent skills in relevant IT systems e.g. Outlook,
* electronic records systems
* Be able to cover the whole geographic area of the city either by possession of a current and valid driving licence or by access to public or other transport
* An ability to fulfil all spoken aspects of the role with confidence through the medium of English
* Have a detailed understanding of key legislation and Guidance underpinning work with adults
* Have a detailed understanding of Adults Safeguarding Principles and best practice
* Have a detailed understanding of relevant council policies and
* procedures
* Have a good understanding of common Health conditions and
* how these can affect the delivery of care.
* Have a detailed working knowledge of Health and Social Care
* systems
* Have a sound knowledge of evidence based social work models, approaches and interventions
* Demonstrate knowledge of Systems thinking and continuous improvement methodology
