The Adult Social Care Community Team are based in a beautiful grade II listed building in the centre of York opposite the train station. Our office provides modern facilities including onsite subsidised café and wonderful views of the world-famous Roman City walls.
It is an exciting time to be part of Adult Social Care in York, with a dynamic journey already underway, to develop the ways in which we work with individuals to enable them to remain independent for as long as possible.
Adult Social Care in the City of York has a clear vision, which lays out a newly developed practice framework, supporting high-quality social work by providing clear expectations within which a consistent approach can be applied. Our practice framework has been developed to support practitioners to use their professional judgement and curiosity, collating the tools, support and guidance practitioners require. We want to empower our staff with an ethos of being flexible and adaptable so we can work inclusively and creatively with our communities.
The Adult Social Care Community Team are a friendly and supportive service working with individuals with physical and sensory impairments from across the City. We are the largest adult social work team in the City of York Council and include several specialist areas such as;

Health specialist
Within the Adult Social Care and Community Team we are recruiting for a review manager to support with the completion of reviews for individuals who are known to adult social care and in receipt of commissioned services. This is a statutory responsibility under the Care Act 2014.
Within this role you will work closely with colleagues in the review team to review and re-assess the needs of adults receiving support from the council, using a person centred, strength based approach to help an individual achieve their outcomes.
To apply you will need good knowledge of adult social care processes and have previous experience working with adults and carers. You will need to be motivated and passionate about making a difference to the lives of people in York. You will also be able to adopt a strengths-based approach and be creative when completing reviews, considering a person’s own network, resources and abilities.

Our approach focuses on the positive attributes of a person's life and of neighbourhoods, recognising the capacity, skills, knowledge and potential that individuals and communities possess.
This role is suitable as a job share opportunity.
Hybrid working arrangements are available for this role. Each role at CYC is different and your working arrangements will be determined based on the requirements of the role and in collaboration with you and your needs as part of the recruitment process. Your contractual location will be the designated council office for your team.
