You’ll work collaboratively with patients, carers, and internal and external partners to ensure safe, timely, and appropriate discharge pathways.You’ll play a vital part in maintaining patient flow, delivering excellent care, and ensuring a positive experience for every patient.If you’re proactive, experienced, and passionate about supporting patients’ journeys through acute care and beyond, we’d love to hear from you. To apply: Please submit your expression of interest and a short statement outlining your suitability for the role to Annette White Annette.White@hcrgcaregroup.com Main Responsibilities Provide a calm, supportive presence on front-door wards to help avoid admissions and support timely discharge.Work collaboratively with social care partners to offer seamless, integrated support.Guide and mentor Band 6 in-reach staff within the acute setting.Champion respectful, patient-centred care.Help reduce length of stay through early, well-coordinated discharge planning.Join ward whiteboard meetings to maintain progress and assist decision-making.Contribute to or lead key reviews related to discharge decisions.Share updates on discharge policies, national guidance, and best practice.Apply sound understanding of the Mental Capacity Act during planning.Remain vigilant to safeguarding concerns and escalate when needed.Use detailed CHC knowledge to support assessments.Oversee Wiltshire discharges, ensuring medications, equipment, and referrals are arranged.Offer clear, supportive communication to patients and carers.Adapt communication style to meet varied needs.Maintain accurate, up-to-date patient records for effective information sharing.Support teams with correct documentation and timely onward referrals.Use multiple electronic patient record systems confidently.Recognise difficult conversations, offer support, and escalate appropriately.Understand the complaints process and assist staff and service users sensitively.Stay updated on national directives affecting discharge and help teams apply them.Maintain awareness of Health & Safety and Infection Prevention in discharge planning.Use a balanced, pragmatic approach to risk management.Work closely with the Patient Flow Hub to resolve barriers to discharge.Participate in system-wide meetings to support onward care planning.Support colleagues who are learning, including those shadowing or newly inducted.Model respectful, positive working relationships with all partners.Encourage carer involvement and initiate Carer’s Assessments when needed.Build supportive relationships in pressured situations and use opportunities to network.Strengthen links with voluntary and third-sector organisations.Prioritise actions that create safe, effective discharge plans, escalating concerns when required.Contribute to internal and national audits.Help shape and develop policies relevant to the role.Adjust plans quickly when patient needs change to keep discharge planning moving.Seek ongoing learning opportunities, both formal and informal.
