Clinical Pharmacist
Band: 7
SPECIFICATIONS:
1. Clinical review of prescriptions and patient-orientated medication review
2. Provision of specialist pharmaceutical advice to multi-disciplinary teams
3. Involvement of patients in decisions about their medicines to support adherence and improve outcomes
4. Potential to utilise non-medical prescribing skills within the role outline
5. Opportunity to influence the development of services to improve patient care through service redesign
6. Participation in clinical audit and service evaluation • Supervision of junior pharmacy staff
7. Contribution to the training and development of junior pharmacy team members and other members of the multi-disciplinary team
8. Participation in the clinical supervision programme
9. Input to the development of policies and procedures and treatment guidelines
10. Contribute to initiatives to improve medicines safety
QUALIFICATIONS & JOB REQUIREMENTS:
11. 6 months of Healthcare experience as a Clinical Pharmacist
12. Right to work in the UK
13. References covering the last 3 years of clinical employment
14. AHPs should have 1 year of experience
15. HCPC registered
16. Overseas Police Check, if any
17. Indemnity Insurance
18. Up-to-date CV
