The Acute Liaison Nurse will act as a clinical expert and professional resource to the multi‑professional team, patients with a learning disability, their families and carers across the Trust.
* The post holder will plan and deliver care in accordance with the Trust’s commitment to deliver high quality care.
* The post holder will work in collaboration with mainstream hospital services to enable open and easy access to healthcare services for people with learning disabilities.
* The post holder will work alongside health professionals and senior hospital staff to achieve a positive experience and clinically appropriate outcomes for patients with a learning disability.
* The post holder will initiate and support person‑centred approaches to the delivery of care within healthcare services.
