Overview
We are looking for an experienced and motivated Band 7 PCN Lead to join our community nursing team, supporting high‑quality care for housebound patients in their own homes. As a Clinical Nurse Specialist, you will provide strong clinical leadership while overseeing the day‑to‑day management of staff and ensuring the delivery of safe, effective and compassionate care across your teams.
Key Responsibilities
* Use advanced community nursing expertise to offer sound clinical judgement, act as a role model and support colleagues in managing complex caseloads.
* Triage new referrals, coordinate care delivery and allocate work to team members with appropriate skills and competencies.
* Lead on problem‑solving, decision‑making and planning care for patients with complex needs.
* Take responsibility for workforce planning and rota management, ensuring staffing levels meet service demand.
* Support and develop the workforce to meet the demands of a busy service, managing day‑to‑day staff issues, promoting wellbeing and ensuring team confidence to deliver safe care.
* Collaborate with Primary Care Networks and the wider MDT to coordinate shared care approaches that meet local health and social care needs.
* Actively contribute to service improvement projects with a strong focus on patient safety, experience and continuous development.
Qualifications
* Band 7 clinical grade in nursing or equivalent experience.
* Advanced community nursing expertise and strong clinical skills.
* Solid understanding of complex disease management.
* Demonstrated leadership experience within a community or primary care setting.
* Ability to plan, organise and manage staff and resources effectively.
* Full UK driving licence or a valid EU licence (if driving is required).
