Description Transformation Manager - Intermediate Care | 9847 | Fixed term contract/secondment opportunity until 30 September 2027 | 37 hours per week | £50,166 to £54,277 per annum | Grade L | Norwich The Transformation Manager supports Adult Social Services in delivering significant Transformation, Continuous Improvement, and Legislative projects and programmes. In this role, you will lead the day-to-day planning and delivery of key activities within the Intermediate Care portfolio, working closely with the project team and key stakeholders to drive progress and deliver outcomes. You will work with stakeholders across the department, council, health partners, providers and service users to identify, evaluate, plan and implement solutions that promote a holistic approach to Adult Social Services. We are looking for someone who has knowledge and experience in the delivery of transformation ideally within the adult social care and/or health sectors and has a passion for innovation. We want someone who is excited to work on transformation projects that ensures our residents have the best outcomes. Key requirements include the ability to collate, analyse and interpret performance data, drive improvements across organisation boundaries, and support digital innovations and transformation.
