Job summary

An exciting opportunity has a risen for a motivated and dynamic Nurse to join our team as a (Trainee) Advanced Care Practitioner working as part of our Integrated Specialist Palliative Care team.

Working in the inpatient, outpatient and community (Virtual Ward) setting, the successful candidate will assess patients, plan, deliver and monitor the care and management of patients with complex specialist Palliative Care needs.

The successful applicant will be expected to demonstrate advanced clinical skills, participate in service evaluation and improvement, audit and research. They will also be expected to lead on education both within the service and externally.There are 3 positions available.

Main duties of the job
* Assess patients and plan, implement and evaluate care using specialist clinical knowledge and skills
* Review patients and provide care in inpatient, outpatient and Virtual Ward setting
* Contribute to multidisciplinary approach to patient management
* Provide education and support to patients, carers and colleagues
* Participate in audit, research and service development within the Specialist Palliative Care service.

* Act as an innovative, enthusiastic role model providing leadership, guidance and advice to staff on operational and professional issues promoting an open and honest and transparent culture.
* To improve the experience and outcomes of care for patients by taking the lead in investigations and treatments prescribed.
* To provide a consistent and personalised service by ensuring that the patients receive the best care, in the best place from the best person or team.
* To provide social, emotional and psychological support for patients from referral and throughout their treatment.
* To deliver expert specialist evidence based care including: development, assessment, planning, implementing, evaluating programmes of evidence based nursing care for a defined caseload.
* To make appropriate referral to consultants/other specialist/members of the multidisciplinary team in primary, secondary or tertiary care settings.
* To be aware of risk management issues both locally and nationally and develop strategies to address identified issues.
