About North Yorkshire Horizons:
North Yorkshire Horizons is a service made up of several key partners including Waythrough, enabling the development of a peer led, recovery community. Providing a safe environment in which people can develop the life skills, and confidence to achieve and maintain recovery and abstinence from their substance misuse, successfully re-integrate into society, and become active, contributing citizens.
About the role:
As directed by the Project Manager, the post holder will be an integral member of the dynamic multi agency team developing and delivering comprehensive and tailored individual interventions that will include delivery of a full range of structured, psychosocial and clinical support. The post calls for close co-operation with other professionals such as GP’s, practice nurses and pharmacists and will involve delivering clinics from host environments/GP practices.
What You’ll Do
1. Working autonomously to manage a caseload of service users, including delivering a range of psychosocial interventions.
2. Working as part of a multi-disciplinary team to undertake ongoing assessments, risk assessments, and appropriate referrals within the community.
3. Facilitating group sessions with a range of individuals of varying backgrounds.
4. Working with partnership agencies to ensure continuity of care and support.
5. Providing duty cover for individuals accessing our services.
To Succeed in This Role, You’ll Need:
6. Experience of managing a caseload.
7. Knowledge and/or experience of substance misuse.
8. Experience supporting vulnerable people, whose lives have been affected by alcohol and/or drug use.
9. A relevant qualification in Health and Social Care.
10. Able to use Microsoft Office programmes efficiently.
