Responsibilities
The Integrated Transfer of Care (ITOC) Team is a dynamic, multi-skilled team comprising nurses, occupational therapists, physiotherapists, and Assistant Practitioners. We collaborate with social services and other Locala community services.
Our aim is to support safe and timely hospital discharges for individuals who are medically fit but no longer need hospital care, helping them transition to more suitable environments. We conduct assessments and provide care tailored to help individuals regain strengths and skills for independent living. We also work to prevent unnecessary admissions from A&E and Frailty departments requiring intervention for discharge.
The ITOC model strives to improve outcomes by:
* Reducing hospital stay durations for those no longer needing acute care
* Assessing individuals in appropriate settings outside the hospital
* Providing multidisciplinary reablement and rehabilitation plans
* Enabling the urgent care system to prioritize acute hospital care for those who need it most
Service Structure:
* The Hospital Discharge Team identifies individuals with ongoing care needs and arranges discharge pathways within the Recovery hub service.
* Home First and Bedded Pathway is the default discharge route, focusing on returning individuals home with necessary support.
Longer-term needs are assessed after stays in the Recovery Hub, with both teams covering A&E and Frailty patients across Kirklees.
Opportunities for broader practice support across Unplanned and Intermediate Care services are available.
