<div><p>THIS JOB IS BASED IN SOUTHAMPTON, HAMPSHIRE</p><p>Are you passionate about palliative and end of life care and looking for a new challenge?</p><p>We are looking for someone who holds a high amount of empathy and compassion, and who can make a real difference – not only to our patients but to their family and support network.</p><p>The postholder will provide support to patients in their own homes or preferred place of care who have palliative or end of life care needs, raising clinical standards and promoting a learning environment for other health care providers.</p><p>They will be responsible for planning, assessing, implementing, and evaluating care in partnership with the Multidisciplinary Teams and out of hours providers.</p><p>A clean driving licence and use of a vehicle with business insurance is essential.</p><p>Part-time: 30 hours a week to be worked flexibly to include days, nights, weekends and bank holidays on a shift rota basis.</p><h3>Main duties of the job</h3><p>The Senior Staff Nurse - Community role will contribute to the provision of specialist palliative and end of life care in the community, being based at Mountbatten Hampshire.</p><p>Under the supervision of the Locality Leads, they will work autonomously and as a member of the Community Palliative Care Team influencing the delivery of ongoing patient care, as well as providing advice and support to patients, their families and other healthcare professions within the community.</p><h3>About us</h3><p>Mountbatten Hampshire is part of the Mountbatten Group, and its sister hospice is Mountbatten Isle of Wight.</p><p>We pride ourselves on providing expert care and support to our communities across Hampshire and the Isle of Wight, who are facing death, dying and bereavement.</p><p>Our vision at Mountbatten is of a world where all dying people and those closest to them, have access to expert care of the highest quality, provided by compassionate and professional teams.</p><p>Our Values</p><p>We care about what we do. We exist for our local population, now and into the future, and we believe that we can achieve more together by working in partnership with others.</p><h3>Job description</h3><h3>Job responsibilities</h3><ul><li>To provide an environment that encourages patients and their families to play an active part in their care enabling them to voice their wishes about current and future care requirements.</li><li>To provide advanced levels of clinical decision‑making through assessment and interpretation of clinical information to support patients, families and carers who have complex or rapidly changing physical symptoms, emotional or practical needs.</li><li>To assess, plan, implement and evaluate patients care needs as required by your designated patients, including admittance to the Hospice/Hospital and advanced care planning including identifying their preferred place of death.</li><li>To provide appropriate pain and symptom management and provide directional support for their psychological and spiritual needs of the patient, their families, and carers.</li><li>To provide support, information and advice to patients, families, colleagues, and other healthcare professionals, including supporting the Mountbatten Care Co‑Ordination Centre.</li><li>Prioritise daily duties in response to patients and families requirements in a manner that maintains and promotes quality, NMC code of practice, the patients care needs and the values of Mountbatten.</li><li>To be fully conversant with organisational medicines policies and palliative care guidelines, algorithms and administer medication in line with these controls procedures.</li><li>To provide training support in the use of syringe drivers and provide teaching to others including, staff nurses and students in collaboration with the Community Team and other professionals.</li><li>In accordance with Mountbatten Infection Control policy and procedures, promote an environment that supports infection prevention and control policies and procedures, ensuring that they and other staff practice in accordance with these documents.</li><li>The postholder will act as an advocate for their patients and families ensuring their views are known to the team including in the case of advance care planning and occasions where offered care is declined.</li></ul><p>Please refer to the attached Job Description and Person Specification for the full duties and responsibilities of this role.</p></div>
