This important post will work alongside a team of Consultant Geriatricians, Community Nurses, Therapists, and Health Care Assistants in the provision of the Frailty Virtual Ward working in the community, in the patient's own home. You will be part of a high profile service to help alleviate current pressures in the NHS. The service reduces demand for inpatient beds and on Emergency Departments and optimises patient experience by enabling them to receive safe clinical specialist care in the comfort of their own home.
If you are a dynamic individual, keen to take on a new challenge and develop in this innovative and unique role we would love to hear from you. You will need experience as a qualified nurse and be able to drive with access to a car for work.

As a newly appointed employee, you are responsible for incurring the cost of your initial DBS check relevant to your post; the amount will be deducted from your first salary with the Trust.
