An opportunity has become available to work within the Care4CE Dementia Reablement service. There are two permanent positions for Grade 6 Senior Support Worker, for 22 hours per week, primarily working Wednesday to Friday 9am – 5pm, although some early evenings may be required. The service is based in Macclesfield Town Hall, but the team are Flexible and Mobile Workers and visit referrals in the community and so can also work from home at times.
ABOUT THE ROLE
The Dementia Reablement team provide short term (up to 12 weeks) support to individuals recently diagnosed with dementia. The team provides information, advice and practical support to enable individuals referred to the service to continue to live independently for as long as possible and to continue to enjoy a good quality of life. The team provides specialist, short term interventions that are designed to equip people with early dementia, and their families, with the practical skills, tools and information that can improve outcomes for the person affected.
