The service provides a stepping stone to independence by supporting people to regain lost skills, learn new skills, and adapt to the challenges of living independently.
You will work within a CQC-rated "Good" Home Support Reablement Council Team, collaborating with professionals such as NHS colleagues, Social Workers, and Occupational Therapists to provide comprehensive care, including moving and handling, personal care, administering medication, and ensuring service users' dignity and wishes are at the centre of care.
