This is an exciting opportunity to make a real difference in the lives of individuals experiencing serious mental health challenges, helping them to live as independently and meaningfully as possible within the community. Assess the needs and circumstances of individuals and families, developing tailored care and support plans. Act as Care Co-ordinator, ensuring care packages are well planned, monitored, and evaluated. Attend Mental Health Tribunals and contribute to care reviews and planning meetings.
