Role Overview
We need experienced permanent Full and Part Time Social Workers to work across Lancashire as part of our Older People & Physical Disabilities Service.
Responsibilities
* Undertake strength-based, outcome-focused assessments and support planning.
* Ensure residents receive the right amount of support.
* Support people to access local services, maintain independence, promote health and wellbeing and live independently.
* Work with other agencies, professionals and informal carers to provide a holistic service.
* Use supportive managers and access training and progression opportunities.
Qualifications
* Registered with Social Work England (SWE) or have applied to register with SWE.
* Experience in social work, relevant to older people and physical disabilities.
