More About Your Role
* Deliver a range of sustainable Healthy Communities projects/programmes that deliver measurable impact for the community across 5 centres in Wokingham Borough.
* Complete monitoring and evaluation requested by your line manager in line with current data protection protocols, including gathering case studies to demonstrate impact.
* Deliver Places Leisure National Healthy Communities Programmes, such as Exercise Referral, Specialist Classes, Falls Prevention, Cardiac Rehabilitation, Cancer Rehabilitation, and other programmes focusing on tackling health inequalities, in line with relevant training/qualifications held.
* Support the coordination of programmes, including processing referrals from healthcare professionals, conducting telephone consultations, and managing administration through the bookings system.
More About You
* Qualified instructor in relevant discipline to deliver sessions independently, e.g., Level 3 GP Referral, Postural Stability Instructor, Level 4 Cardiac Phase IV, Level 4 Cancer Rehabilitation, Level 4 Neurological Conditions.
* Experienced in planning, delivering, and monitoring sessions.
* Passionate about improving the health of the local community.
* Proficient in using MS tools.
* Confident in interacting with clients in person and via telephone.
