Learning Disabilities Social Worker - Young Adults Team (16-25) Worcestershire County Council Up to £32 per hour Are you a passionate and experienced Social Worker ready to make a real difference in the lives of young people with disabilities as they transition into adulthood? Worcestershire County Council is seeking a highly skilled and motivated Learning Disabilities Social Worker to join our dedicated Young Adults Team (16-25) within the All Age Disability (AAD) Service. The Role This is an exciting opportunity for a Social Worker with specialist skills across both Adult Services and Children’s Services to work with young people aged 16-25 with Learning Disabilities, and sometimes those with physical disabilities or complex health needs. The AAD Service is committed to supporting young people aged 0-25 with SEN and additional needs. As a key member of the Young Adults Team, your work will focus on Preparation for Adulthood across four key areas: * Education/Employment * Independent Living * Community Inclusion * Health Your work will be varied and you will be essential in helping young people successfully transition from Children’s Services into their adult lives. This requires a strong, multi-agency approach, working closely with: * Worcestershire Children’s Services and Adult Services * Special Educational Needs Department (SEND) * Health colleagues * Schools and Colleges What You'll Need * A recognised Social Work qualification and current registration. * Essential experience of working within transitions (Children to Adult Services). * Expertise in and a solid understanding of both Children’s and Adult legislation. * The ability to effectively use case management systems (experience with Liquidlogic in both children's and adult systems is highly desirable). * The ability to drive and use of a car for business purposes, as the role covers all of Worcestershire and includes some national placements.
