An exciting opportunity has arisen for a Community Staff Nurse to join Crawley Healthy & First Community PCN.
The PCN is a group of practices working together in collaboration with community and other health and care services. PCNs enable greater provision of proactive, personalised, coordinated, and more integrated health and social care. Care is organised around individuals' identified needs through collaborative working by service providers to deliver excellent patient care at the heart of the community.
The Crawley Healthy & First Community PCN Team is looking for registered nurses to join. Whether you are newly registered or seeking a new challenge, community nursing offers a highly diverse and rewarding field.
Community nursing roles include various specialties, and successful applicants will have an individual induction plan to support their transition into this environment.
The role involves delivering high-quality care in patients' homes, supporting rehabilitation, self-management, and independence across the continuum of care. You will work within a team supporting evidence-based care, staff learning, and development. Key responsibilities include diabetic management, wound care (including leg ulcers), palliative care (working closely with hospices and GPs), bladder and bowel care (including catheter management), and other continence needs.
