Overview
Warrington and Halton Teaching Hospitals NHS Foundation Trust is offering an opportunity within the Acute Care Team. The team supports medical and nursing staff in caring for deteriorating patients and those transitioning from Intensive care to wards. The role encompasses critical care outreach, acute kidney injury specialist nursing and site management. The post is suitable for nurses with extensive experience caring for critically ill patients, with formal and bedside training available to support existing clinical expertise.
This post is ideal for skilled practitioners who are passionate about patient safety and clinically effective care for patients at risk of deterioration and those post critical illness. Experience in clinical assessment and treatment of acutely ill patients in an acute setting is essential, as is a questioning nature and keen eye for detail. The successful candidate should work well within a team and be willing to work a mixed shift pattern including days, weekends and nights.
Main duties
* Attend clinical emergencies and provide care to deteriorating adult inpatients
* Review patients with acute kidney injuries, working closely with renal consultants
* Autonomously provide clinical review, including physical examination, assessment and diagnosis of deteriorating patients
* Review critical care patients post-discharge, co-ordinating MDT care to maximise opportunities for physical and emotional rehabilitation
* Maintain accurate patient documentation in line with professional body requirements for all patients reviewed or where advice has been provided
* Provide site management out of hours in conjunction with the off-site on-call manager, allocate staffing resources, manage incidents and support staff, patients and their relatives
The post holder is required to have excellent interpersonal skills with the ability to liaise effectively with the multidisciplinary team and to act as an excellent role model, maintaining a positive workplace culture. They will be expected to work in partnership with a variety of nursing and medical teams across the Trust.
