Our Team:

We are looking to recruit a Community Care Worker, to work within our Halton Intermediate Care and Frailty Service.

Halton Intermediate Care and Frailty Service is a multi disciplinary team consisting of Social Care Workers, Therapists and Nurses, the role of Community Care Worker will be to work within the Adult Social Work Team who are responsible for the delivery of person centred support and an asset/strengths based support planning for Adults over the age of 18 years within a MDT setting. The focus of the role will be supporting Adult Social Care with the completion of social care reviews and assessments and participation in MDT's.

Halton Borough council's expectations are high and you will need a strong commitment to providing first class person centred care to Halton residents over 18 and support to their carers.

You will be passionate about working with a range of service user groups (older people, adults with learning disabilities and adults with physical and sensory difficulties, challenging discrimination whilst promoting independence.

You will have a recognised Social Care Experience, within a Care Management team or a social Care setting, good interpersonal and communication skills, a flexible approach and a willingness to continually improve your practice; in addition, you must hold a full driving licence.
