Nottingham University Hospital NHS Trust seeks applications for a substantive CESR Fellow in Acute Medicine based at the Queens Medical Centre Campus. Our CESR workforce currently has 5 fellows, and we aim to expand this team.
The CESR, or Certificate of Eligibility for Specialist Registration, is the route to specialist registration for doctors who have not completed a GMC-approved deanery training programme but can demonstrate that their specialist training, qualifications, and experience are equivalent to the UK CCT requirements. It is a competency-based programme that allows trainees to compile a portfolio of evidence to meet the Acute Medicine CCT curriculum. Successful completion grants entry onto the Specialist Register, enabling doctors with CESR to apply for substantive consultant posts in the UK.
This post is ideal for doctors aspiring to a career in Acute Medicine within one of the largest and busiest departments in the country. It offers an excellent training opportunity for experienced post-CT SHO, Clinical Fellows, or Registrars seeking a career outside a deanery-based training programme.
Post-holders will manage a full range of adult patients under consultant supervision. The Acute Medicine middle-grade rota includes three roles:
1. B3 SpR – Responsible for senior review of new admissions, supporting junior doctors, performing clinical procedures, overseeing patient intake, expediting discharges, and maintaining patient flow.
2. AMRA SpR – Responsible for senior review of GP admissions and Same Day Emergency Care patients, supporting junior staff, performing procedures, and ensuring smooth patient flow.
3. AMRAT SpR – Manages referrals from the Emergency Department, ensuring correct pathways, and providing advice to ED colleagues.
On-call duties cover both campuses and include GIM on-call, stroke thrombolysis, support for the medical Level 1 unit, and respiratory care at Levels 1/2.

The post involves working under clinical direction across all areas of the Acute Medicine Department, with rotations potentially on other specialties. Typical weekly hours are approximately 42-46, including out-of-hours work on the GIM rota, with potential progression to the consultant on-call rota upon competency.
Successful applicants will join a multidisciplinary team committed to high-quality patient care and training. Our Acute Medicine Unit is undergoing significant expansion and modernization, with three clinical areas:
* Acute Medicine Receiving Area (AMRA) – 20 beds for community and ED patients
* Acute Medicine Assessment Unit (ward B3) – 64 beds for assessment and short stays
* Acute Medicine Level 1 Unit – 12 beds for high-acuity admissions
We also offer a Telephone Triage, Advice, and Navigation service for GPs, improving patient pathways and reducing unnecessary admissions.
