About the Role

We are seeking a dynamic and experienced Continuing Healthcare (CHC) Funding Lead to join our Adult Social Care CHC team. This pivotal role will lead on complex funding arrangements, including CHC, joint funding, and Section 117 aftercare, ensuring robust decision-making and compliance with national frameworks.


Key Responsibilities

* Lead and coordinate health funding processes across health and social care.
* Represent Southampton City Council at CHC and joint funding panels, multi-agency panels and strategic forums.
* Provide expert guidance on joint funding and Section 117 arrangements.
* Supervise and support a team of professionals, fostering a collaborative and high-performing environment.
* Work closely with commissioning, finance, and operational teams to ensure effective use of resources.
* Ensure compliance with the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care.


Essential Criteria

* Registered professional: Social Worker, Nurse, or Occupational Therapist.
* Extensive experience in CHC, joint funding, and Section 117 processes.
* Proven track record of supervising and mentoring other professionals.
* Strong understanding of legislation and policy relating to health and social care funding.
* Excellent communication, negotiation, and stakeholder engagement skills.


Desirable

* Experience working in integrated health and social care settings.
* Knowledge of commissioning and budget management.


About the team

You will join an energetic and supportive team striving to support people in Southampton live active, safe and independent lives. The Adult Social Care CHC Team represents Southampton City Council Adult Social Care at Decision Support Tool (DST) Multi-Disciplinary Team (MDT) meetings to determine NHS Continuing Healthcare (CHC) eligibility. The team works collaboratively with health partners to agree joint funding arrangements, including Section 117 aftercare (in liaison with relevant adult social care teams), and provides expert advice to social care teams on complex health and care needs. Their role is central to ensuring integrated, person-centred support and fostering strong inter-agency relationships.

Salary: The salary band for this role is £47,181 - £51,357.The starting salary is £47,181 with annual progression through the salary bands.
