4 - 7 sessions per week
within a well-established multidisciplinary team dedicated to delivering high-quality patient care. Key Duties and Areas of Responsibility In accordance with the practicerota, undertake duties including surgery consultations, triage, telephoneconsultations and queries. Make professional, autonomousdecisions in relation to presenting problems, whether self-referred or referredfrom other health care workers within the organisation. Screen patients for disease riskfactors and early signs of illness. Assess the health care needs ofpatients with undifferentiated and undiagnosed problems. Visiting patients at home,checking and signing repeat prescriptions as well as dealing with paperwork andadministration in a timely fashion. Perform clinical examinations ofpatients to assess, diagnose and monitor conditions. Carry out minor surgery whererequired. Provide emergency care while waitingfor emergency transfer. Effective management of long-termconditions. Working collaboratively, acceptingan equal share of the practice workload. Maintain accurate clinical recordsin conjunction with good practice, policy, and guidance. Undertake telephone triage andduty doctor roles. Liaise with other healthcareprofessionals as part of multidisciplinary teams. Develop care plans for health inconsultation with patients and in line with current practice disease managementprotocols. Record clear and contemporaneousconsultation notes to agreed standards. Prescribe in accordance with thepractice prescribing formulary (or generically) whenever this is clinicallyappropriate. Participate in practice audits as andwhen required. Participate in local initiativesto enhance service delivery and patient care. Participate in the review ofsignificant and near-miss events applying a structured approach i.e., rootcause analysis (RCA). Adhere to best practicerecommended through clinical guidelines and the audit process. Contribute to the successfulimplementation of continuous improvement and quality initiatives within thepractice. Contribute effective to thedevelopment and maintenance of the practice including clinical governance,training, financial management and HR. Prepare and complete therevalidation process.
