Community Advanced Health Care Assistant – Short Term Care Team

This role supports patients when leaving hospital who require short term care and rehabilitation. The service provides care support and re‑enablement for patients and their families across seven days a week.
The advanced community health care assistant will work in collaboration with district nurses, locality coordinators, discharge teams and community therapy services to promote functional independence and rehabilitation for short term care and support needs.
The assistant will undertake administration of medication and must have appropriate training. Training will be provided in line with local policy if not achieved at application stage.
Evidence of the NVQ qualification will be required on the day of the interview.
Main duties of the job
* Assist with patient personal care and support needs, up to 4 visits a day over 7 days a week.
* Care for patients who have recently been discharged from hospital and are returning home with changed care and support needs.
* Promote independence wherever possible to improve quality of life.
* Provide physical care with respect, promote dignity, and secure consent for activities.
* Maintain a high standard of care, adapt to individual and family needs, and be resourceful when required.
Person Specification
Education and qualifications
* Must hold NVQ or NQF equivalent Level 3 training in Health Care and Social Care.
* IT skills.
* Evidence of recent study or development.
Knowledge, Training & Experience
* Minimum 6 months experience working in a health or social care setting.
* Hold a driving licence.
