About the role: You will join our valued team of Case Managers to support young people and adults following life-changing injuries. As a Case Manager, your focus is the client’s best interests, coordinating care and rehabilitation tailored to their specific needs. The aim is to enable clients to regain and maintain the best quality of life by working closely with them, their family, and care team to support their physical, cognitive, and emotional wellbeing.
The role is home-based, managing a client caseload with visits within your geographical area. All ILS Case Managers have the option for part-time salaried or flexible hourly employment, from 21 to 30 hours per week. A driver with a car is preferred, as travel up to 2 hours from your home is expected; travel and mileage are paid.
