We are seeking a compassionate and detail-oriented Case Worker to join our team. The successful candidate will play a crucial role in assessing client needs, developing care plans, and ensuring the effective delivery of home care services. This role requires excellent communication, problem-solving, and organizational skills to support both clients and care staff.
Key Responsibilities:
1. Client Assessments: Conduct initial and ongoing assessments of clients to determine their care needs, preferences, and risks.
2. Care Planning: Develop, implement, and review individualized care plans in collaboration with clients, families, and care teams.
3. Liaison & Advocacy: Act as a key point of contact for clients, families, care workers, and external professionals (e.g., social workers, healthcare teams).
4. Compliance & Safeguarding: Ensure that all care services align with CQC regulations, company policies, and safeguarding protocols.
5. Monitoring & Quality Assurance: Regularly review care plans, conduct visits, and gather feedback to ensure the highest standards of care.
6. Staff Support & Training: Provide guidance and support to care workers, ensuring they understand and follow client care plans effectively.
7. Crisis & Risk Management: Address any urgent client concerns, respond to safeguarding issues, and report incidents in a timely manner.
8. Administrative Duties: Maintain accurate records, update client files, and ensure documentation is completed in compliance with regulatory requirements.
Requirements:
* Previous experience in a case worker, care coordinator, or similar role within the health and social care sector.
* Minimum Level 3 in Health and Social Care.
* Strong understanding of domiciliary care, person-centered planning, and safeguarding principles.
* Excellent communication and interpersonal skills.
* Strong problem-solving and decision-making abilities.
* Knowledge of CQC standards and compliance requirements.
* Ability to work independently and collaboratively within a team.
* Full UK driving license and access to a vehicle (preferred).
* NVQ Level 3 or higher in Health & Social Care (desirable).
