Lead Practitioners Role
Lead Practitioners provide leadership and support to a team of staff responsible for delivering the Directorate’s statutory duties related to adult social care. The role includes accountability for overseeing performance, assessment, and risk management.
The postholder will be skilled at embedding new ways of thinking and working, developing strong relationships with commissioners, teams, and partners. They will also seek efficiencies across their areas of responsibility.
Key Duties/Accountabilities
* Lead and manage teams delivering personalized social care, achieving positive outcomes through an enabling culture of positive risk management.
* Coach and mentor colleagues to embed personalisation and community-based strengths and assets.
* Demonstrate the Council's Core Abilities at the relevant level.
* Exhibit personal leadership and political and commercial astuteness.
* Support a high-performing, flexible workforce.
* Act as a role model, managing uncertainty and responding creatively to change.
* Assess service user needs, agree outcomes, and review them; manage a small complex caseload.
* Oversee complex case decision-making, ensuring safeguarding policies are followed to protect adults at risk, promoting a personalized safeguarding approach.
* Contribute to service planning and commissioning through contact with providers.
* Support statutory obligations and performance indicators at local and national levels.
* Implement adult care quality assurance systems, monitoring casework and ensuring compliance with equal opportunities policies.
* Provide information on individuals and staff impacting statutory duties.
* Monitor expenditure within delegated budgets.
* Allocate workloads, monitor case files, and manage case management issues.
* Chair BIA meetings, oversee safeguarding processes, and handle complex legal liaison, including risk assessments and complaints.
* Develop staff through training, coaching, and mentoring to meet performance standards and address quality issues.
Essential Experience Required
* At least 2 years of post-qualification practice experience in health or social care.
* Current HCPC or NMC registration.
* A professional qualification in Social Work, Nursing, or Occupational Therapy.
