Responsibilities
* A Community Specialist Practitioner who combines delivering clinical excellence with leadership responsibilities.
* Assess and attend to the diverse health needs of the community, including end-of-life care and patients with long-term or complex conditions, under the guidance of the Nursing Team Leader.
* Provide care in patients\' own environments, blending hands-on clinical work with nurturing emerging nursing talent and ensuring safety and quality of care.
* Work within a wider integrated neighbourhood team as part of an MDT approach to address wellbeing needs.
* Utilise your CSP qualification and community nursing experience to deliver patient-centred care aligned with our values: pride, respect, empathy, consideration, compassion and dignity.
What We Look For
Essential
* First Level Registered Nurse
* Community Specialist Practitioner (CSP) qualification
* Registered Non-Medical Prescriber
Desirable
* Masters level education
* Practice teacher training
* Other relevant courses (e.g., care of the elderly/palliative care/management)
Experience
* Evidence of continuous professional development
* Evidence of participation in clinical audit and implementation of audit recommendations
* Excellent communication, diplomacy and negotiation skills
* Leadership skills and experience in change management
* Teaching skills and ability to implement change
Skills
* Highly developed clinical practice
* IT skills
* Awareness of wider NHS agenda and national/local strategic direction/frameworks
* Full, current driving licence and access to a car
