The Role
The post holder will support the District Nurse in delivering effective, holistic nursing care to a designated caseload within Inverclyde Health and Social Care Partnership (HSCP). The role contributes to the assessment, planning, implementation, and evaluation of care for individuals with a range of healthcare needs in the community setting. The Community Nursing Service provides high‑quality nursing care to the housebound population of Inverclyde, delivering safe, effective, person‑centred care within patients’ own homes, supporting independence, dignity, and wellbeing in line with NHS Greater Glasgow & Clyde values.
Evidence of current NMC registration must be provided prior to the start date. PVG Scheme membership / Disclosure Scotland check required. Compliance with NHS Greater Glasgow & Clyde pre‑employment checks.

Duration, Location, and Working Pattern
* Location based in Inverclyde HSCP
* Permanent contract
* Full‑time position (36 hours per week)
* Shift pattern: 5 days over 7, including weekends, evenings and public holidays
Key Responsibilities
* Assess, plan and implement programmes of care as directed by the district nurse, in partnership with patients, family, carers and significant others, using evidence‑based approaches that consider lifestyle, cultural background and other factors impacting health.
* Assume the caseload holder role during short periods of sickness, absence or annual leave, seeking support as required.
* Utilise clinical and environmental risk assessment tools to identify risks and implement appropriate interventions.
* Report and participate in the critical appraisal of incidents to reduce recurrences and support further investigation.
* Support patients, families and carers to achieve optimum health and independence.
* Plan and prioritise workload, delegate work where appropriate and make decisions to ensure patient needs are met.
* Work within systems to ensure continuation of a quality 24‑hour nursing service.
* Provide care in a Treatment Room or Health Centre setting where required.
* Ensure implementation of local, regional and national policies, procedures and guidelines.
* Provide clinical leadership and model the values and beliefs of nursing and the organisation.
* Manage day‑to‑day workload as delegated by the district nursing charge nurse.
Essential Qualifications
* First‑level registered nurse (adult) with relevant post‑registration experience.
* Current registration with the Nursing and Midwifery Council (NMC).
* Ability to work independently and as part of a multidisciplinary team.
* Excellent communication and interpersonal skills.
* Full UK driving licence and access to a car.

* Previous community nursing experience.
