Overview
We are excited to offer a Band 7 Pharmacist position at Glangwili General Hospital, working across our Clinical Decisions Unit (CDU) and Emergency Department (ED) on a permanent basis. This is a high impact role ideal for someone with strong clinical skills, excellent communication, and a pro-active approach to patient care in a fast-paced environment. You will deliver a high quality, patient focused clinical pharmacy service to CDU and ED by carrying out comprehensive medicines reconciliation, provide expert advice on the safe, effective and appropriate use of medicines, assess prescriptions for clinical appropriateness and support prescribers in complex decision-making, supply medication in a timely manner and counsel patients on their medicines. You will be expected to actively participate in board and ward rounds to promote safe prescribing practices and facilitate discharge planning. You will support the Lead Pharmacist for Admissions by contributing to the development and implementation of guidelines and protocols, participate in clinical audit, incident reviews and quality improvement project to enhance patient care.
Responsibilities
* Provide expert clinical pharmacy support to CDU and ED
* Work closely with multi-disciplinary teams to ensure safe and effective medicines use
* Support antimicrobial stewardship and medicines optimisation initiatives
* Contribute to service development and quality improvement projects
* Provide education and training by supervising and mentoring junior pharmacists, pharmacy technicians, pharmacy students as well as clinical ward staff
