We are looking for a dedicated, compassionate and professional Band 6 Occupational Therapist who wants to make a genuine difference in the lives of people working within our Community Therapy South Team.

Main Duties of the Job

What You'll Do

* Provide person‑centred rehabilitation therapy in clients' homes and community settings
* Support clients to develop daily living skills, mobility, confidence, and independence
* Deliver structured intervention plans and monitor progress
* Work collaboratively with physiotherapists, OTs, nurses and wider MDTs
* Facilitate group‑based rehabilitation, wellbeing sessions, or community outreach and in‑reach
* Promote safety, dignity, and empowerment in every aspect of care

Job Responsibilities

Work within and adhere to the College of Occupational Therapists (COT) Code of Ethics and Professional Conduct, The Health and Care Professionals Council (HCPC) standards of practice, The Trust relevant policies and guidelines. The post holder is an autonomous practitioner, responsible for provision of highly specialised assessment, treatment, intervention and discharge planning of clients referred to the service. Responsible for managing a caseload of patients with a variety of complex conditions including chronic, disabling and life‑limiting illnesses. Carry out specialist assessment and treatment including the prescription of essential equipment as part of the therapeutic intervention. Specifically assessing issues related to the barriers which prevent the individual carrying out activities of daily living e.g. mobility, moving and handling, functional restrictions, activity tolerance, hand‑eye coordination, positioning, cognitive impairment, social and psychological issues, environmental restrictions. The post holder will act as a source of expertise and demonstrate advanced knowledge, skill and experience in their role. Assist the Clinical Specialist in planning, coordinating, delivering and evaluating the therapy service. Educate other health care professionals, multi‑agency professionals and therapy students within the speciality. Work in the community as a lone practitioner in line with the Trust Lone Worker Policy. The post holder may be responsible for deputising for the Clinical Specialist as appropriate.


Education and Qualification

* Degree or Diploma in Occupational Therapy
* HCPC Registration
* Accredited fieldwork educator


Experience and Knowledge

* Post registration experience in the relevant clinical area
* Knowledge of current best practice
* An understanding of the principles of clinical governance
* Understanding of professional conduct and confidentiality
* Knowledge of relevant legislation, eligibility criteria/policies and procedures
* Experience of supervising junior / support staff
* Experience of delivering training to colleagues in health and/or other agencies
* Experience of research, audit, reflective practice or outcome measures
* Knowledge of NHS systems
* Previous use of computer based data collection system


Skills and Ability

* Portfolio of documented evidence of Continuing Professional Development
* Able to relate theory to complex clinical situations
* Caseload Management Skills
* Clinical planning and organisational skills
* Able to plan and conduct assessment and intervention
* Ability to identify priorities and meet deadlinesA knowledge and practical experience of manual handling
* Ability to work alone as well as part of a team
* Able to take initiative for development purposes
* Ability to work under pressure and recognise when support is required
* Implement judgement and analytical skills
* Ability to make independent clinical and professional decisions taking into consideration Trust/Departmental Policies/Procedures
* Experience of project work at departmental level
* Public speaking/presentation skills
* Ability to lead in a given situation
* Hold an up‑to‑date UK driving licence and have access to a vehicle or able to make alternative transport arrangements
* Experience of leading a team or service
* An awareness of current NHS trends which influence service provision
