Role summary
This combined role delivers safe, high-quality nursing care in a residential setting while embedding Open Dialogue (OD) relational practice. The post-holder is a registered nurse who provides clinical leadership and person-centred nursing care, and actively works as a relational practitioner—facilitating network meetings, promoting dialogic practice, and supporting staff, residents and families to co-create care through reflective, network-based approaches.
Key Responsibilities
Clinical nursing & leadership
* Provide direct nursing care to residents in line with professional standards and the home's policies (assessment, care planning, medication administration, wound care, continence, end-of-life care).
* Lead and coordinate clinical care for allocated residents; oversee nursing support staff and delegate safely.
* Undertake comprehensive nursing assessments, develop and review individualized nursing care plans, and evaluate outcomes.
* Maintain accurate clinical records, produce reports, and ensure documentation meets legal, regulatory and audit standards.
* Monitor and escalate clinical deterioration; liaise with GPs, specialist services and other healthcare providers.
* Ensure safe medicines management and adherence to infection prevention and control standards.
* Participate in clinical supervision, appraisals and mandatory training; support staff development and competency assessment.
Develop and maintain individual care plans in partnership with the people we care for, their families and friends and other professionals involved in their care. Working across Health Care services, in line with local shared care protocols.
Open Dialogue relational practice
* Work as a relational/OD practitioner: convene, facilitate and participate in network meetings involving residents, families, carers and relevant staff/professionals.
* Hold, tolerate and model uncertainty; promote polyphony (multiple voices), transparency and shared meaning-making.
* Co-create shared formulations and person-centred care decisions with networks, integrating clinical risk management and resident preferences.
* Provide reflective practice, coaching and informal supervision to staff to develop dialogic skills and relational confidence.
* Support implementation of OD principles across the home: contribute to training, practice development and embedding dialogic documentation.
* Ensure relational approaches inform care planning, behaviour support, and safeguarding responses.
Safeguarding, risk & governance
* Act as a mandated reporter for safeguarding concerns; follow local safeguarding procedures and ensure timely referrals.
* Conduct collaborative risk assessments with residents and networks; implement and review safety plans.
* Participate in quality assurance, audits, incident reviews and learning from events; implement agreed actions.
* Maintain confidentiality and consent principles while promoting open communication with families and professionals.
-Monitor and evaluate the effectiveness of care plans and interventions, making adjustments as needed to ensure positive outcomes for service users.
* Ensure compliance with regulatory standards and best practice guidelines, including CQC requirements and relevant legislation.
Multi-disciplinary & stakeholder working
* Work collaboratively with multidisciplinary teams (social workers, therapists, GP, community mental health teams) to coordinate care.
* Promote family and resident involvement in care reviews and service development.
* Build relationships with external agencies to support integrated care and access to specialist services.
Person specification
Qualifications & registration
* Registered Nurse (RMN, LD) with NMC registration (or RN with Mental Health experience) registration experience in older adult nursing or complex care preferred.
* Evidence of training/experience in Open Dialogue, systemic or dialogic practice (e.g., accredited OD course, workshops) or commitment to undertake OD training.
* Current CPR, safeguarding, and any service-specific mandatory training.
Experience & skills
* Proven clinical nursing experience in a nursing/residential care setting, with experience of complex care and medicines management.
* Ability to lead and supervise nursing/support staff; strong clinical decision-making.
* Competent facilitator of conversations and meetings; comfortable working with multiple voices and emotional complexity.
* Good written and verbal communication; competent record keeping and basic IT skills.
* Empathy, cultural sensitivity, availability to hold reflective, non-directive conversations.
Desirable
* Experience facilitating Open Dialogue network meetings or family/systemic work.
* Experience in quality improvement, implementation or training delivery.
* Lived experience of mental health or caring roles (as peer worker or similar).
Checks & other requirements
* Enhanced DBS (or local equivalent) / satisfactory pre-employment checks.
* Flexible working including occasional evenings/on-call to support network meetings or clinical needs.
* Ability to travel between sites as required.
* Commitment to ongoing professional development in nursing and OD relational practice.
Working conditions
* Combination of clinical duties and protected time for OD facilitation, reflective practice groups and training activities.
* Access to regular clinical and OD supervision.
