Offer description

We are seeking an enthusiastic and motivatedindividual who has proven experience within a primary care setting and proficientin the use of Docman and Emis web to undertake clinical coding ofhospital/clinic and miscellaneous correspondence within the practice. Using SNOMEDcoding to code relevant information into patients' electronic healthcarerecords on the clinical system, in an accurate, timely, safe and efficientmanner.

In addition, the post holder will be a teamplayer who can use their initiative to prioritise their workload and supportthe team, providing administrative support, in line with the needs of theorganisation.

The job role will e on a part time basis as part of a job share.


Main duties of the job

Clinical Coding

Accurately code clinical information from GP and hospital correspondence, pathology results, discharge summaries, and consultation notes using SNOMED CT.

Apply codes to reflect diagnoses, procedures, investigations, and relevant health indicators.

Ensure coded data supports effective patient recall, disease registers, and reporting tools.

Work with GPs and clinicians to clarify unclear terminology and ensure accurate record keeping.

Coordinate and manage all administrative tasks related to death processing, liaising closely with GPs and the Medical Examiner to ensure timely and accurate completion of required documentation and processes in line with NHS protocols and legal requirements.

Review incoming correspondence and clinical letters, accurately identify any actions required, and promptly allocate these to the appropriate member of the practice team (e.g., GP, nurse, pharmacist, or administrative staff) to ensure timely follow-up and resolution.

Care Navigation and Patient Support

Liaise with external services (community teams, social care, PCN services) to ensure integrated and coordinated patient care.

Identify and flag vulnerable patients, including those on safeguarding or frailty registers, ensuring appropriate support and escalation.

Data Quality and Record Maintenance

Ensure patient records are accurate, consistent, and updated in a timely manner.

Job responsibilities

JobSummary:

TheMedical Coder is responsible for accurately reading, interpreting, and codingclinical documentation in patient records in accordance with current NHS codingstandards (e.g., SNOMED CT). Working closely with clinicians and administrativeteams, the post-holder ensures the accuracy of coded data used for patientcare, chronic disease management, clinical audits, QOF, IIF, and statutoryreporting.

This isa key role in supporting high-quality clinical data recording, improvingpatient outcomes, and ensuring that the practice meets its contractualobligations and reporting requirements.

Knowledge and Skills

* Previous experience in clinical coding within a GP practice or healthcare environment.
* Knowledge of SNOMED CT coding and experience using a GP clinical system (e.g., EMIS Web, SystmOne).
* High attention to detail and accuracy in interpreting clinical documentation.
* Understanding of QOF, long-term condition coding, and disease registers.
* Good IT and data management skills, including Microsoft Office.
* Commitment to confidentiality and NHS information governance policies.
* NHS-recognised clinical coding qualification or evidence of relevant training.
* Experience supporting QOF and NHS performance reporting.
* Familiarity with read codes (legacy), terminology changes, and reporting tools (e.g., Ardens, Eclipse, or similar).
* Knowledge of NHS policies related to data protection, GDPR, and record management.
