Support Navigator or Support Worker
Location: Bristol
Salary: £24,784 to £31,393 per annum
Job Type: Full-time, Permanent
Shifts: Tuesday to Saturday or Sunday to Thursday
Hours: 37.5 per week - 9am to 5pm or 10am to 6pm
Other info: You can pick up sleep-in shifts but these are not mandatory

We are seeking a dedicated and empathetic individual to join our team as a Support Navigator or Support Worker.
In this vital role, you will work across our service locations and outreach areas to empower and enable service users to achieve recovery, wellbeing, and independence, ensuring they have the tools and resilience needed to thrive.
What You'll Do
As a Support Navigator in Bristol, you will oversee a caseload of service users and provide tailored, person-centred support. Your day-to-day will include:
- Completing, actioning, and reviewing risk management plans and support plans with clear outcomes that enable service users to move towards independence and belonging.
- Managing individual and communal budgets and finances, as well as providing help with the upkeep of a client's property.
- Building strong relationships with family members and actively communicating with external healthcare professionals to ensure a seamless support network.
- Enabling service users to access and participate in their local community, including education and employment opportunities, while working to meet monthly KPI targets.
- Supporting crisis mitigation, managing client engagement strategies, and raising safeguarding referrals when required.
About You
If you have a background in social care, mental health, or community support, we want to hear from you!
What we are looking for:
- Proven experience in a support worker role, with previous experience in health, wellbeing, or mental health sectors highly preferred.
- An NVQ in Health and Social Care is preferred, along with strong IT skills and effective interpersonal skills to build strong relationships with service users.
- The ability to develop and maintain an understanding of the Mental Health Act and medication regimes.
- A compassionate, non-judgmental approach with the ability to consistently demonstrate core service values.
- A valid UK driving license (required), enhanced DBS clearance, and existing Right to Work status in the UK (no visa sponsorship is available for this specific service).
