Main duties of the job
1. Work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
2. Uphold a prescribing qualification, or training to become a prescriber, and work with and alongside the multi-disciplinary team across the Primary Care Network.
3. Take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex poly-pharmacy, those with multiple long-term conditions (in particular COPD and asthma), and people with learning disabilities.
4. Provide leadership on person-centred medicines optimisation, including ensuring prescribers in the practice conserve antibiotics in line with local guidance and quality improvement, while contributing to the quality and outcomes framework and enhanced services.
5. Work within the policies of the scheme and practices.
6. Maintain a good working knowledge of health and safety procedures.
7. Promote patient involvement in the management of the service.
8. Participate in regular supervision sessions and appraisals with your line manager. Attend training and development activities as identified and participate in meetings as required.

Job responsibilities
As a Clinical Pharmacist, you will support the delivery of the new Network Contract DES Service specifications. This includes addressing over-medication, inappropriate antibiotic use, withdrawing medicines no longer needed through NHS-led programmes such as low-priority prescribing, and medicines optimisation more broadly.

* An appreciation of the nature of GPs and general practices, primary care prescribing, rational prescribing concepts, and strategies for improving prescribing.
* Excellent interpersonal, influencing, and negotiating skills.
* Strong written and verbal communication skills, capable of conveying complex and sensitive information understandably to diverse audiences.
* Ability to plan, manage, monitor, advise, and review medicine optimisation issues for long-term conditions.
* Proficient IT skills, including EMIS, Docman, and eConsult.
* Ability to analyze complex technical information and recognize priorities in problem-solving, referring to seniors or GPs when necessary.
* Ability to work under pressure and meet deadlines, producing timely reports.
* Capability to gain acceptance for recommendations and motivate compliance despite barriers.
* Effective independent and team working skills.
* Demonstrates accountability for professional service delivery.
Qualifications
* Mandatory registration with the GPhC, membership of the Royal Pharmaceutical Society, and working towards Faculty membership.
* Masters degree in pharmacy (MPharm) or equivalent.
* Post-graduate diploma level or equivalent training/experience in specialist knowledge.
* Independent prescriber or working towards this qualification.
