Job overview

Could you be part of an innovative, caring and agile Community Nursing team? Do you want to work as part of an award‑winning team?

Welwyn and Hatfield Integrated Care Team are winners of the Hertfordshire Community Trust Leading Lights awards for Demonstrating Fantastic Patient Care.

We are looking to recruit a dynamic and motivated nurse with proven community experience to lead and supervise the team in delivering high‑quality nursing care across the Welwyn and Hatfield locality to prevent admission to hospital, facilitate discharge, deliver planned nursing intervention and manage long‑term conditions.

The successful candidate would be supported to develop skills and join a team who are committed to demonstrating the trust values, caring for each other as well as their patients. We work collaboratively with our partnership organisations across East and North ICB, Hertfordshire Foundation Trust and Hertfordshire County Council.


Main duties of the job

The successful candidate will be:

* Part of a large integrated community team of Band 7, Band 6, Band 5, Band 4 and Band 3 nurses.
* Demonstrate an understanding of current professional and community issues.
* Must have commitment and flexibility to be innovative and responsive to continual developments within community nursing services.
* Must be able to work autonomously and possess excellent communication skills.
* Have a full valid driving licence and access to a car for business purposes (unless you have a disability as defined by the Equality Act 2010).

Detailed job description and main responsibilities

* Provide support to a multidisciplinary team providing clinical, health and social care in admission prevention / avoidance service.
* Set and maintain standards of care and, if required, deploy, coordinate and supervise the nursing team and learners.
* Be responsible for assessment, planning, implementation and evaluation of individualised nursing care to community patients.
* Maintain accurate patient records.
* Participate in long‑term management of patients in the locality.
* Have continued responsibility for a defined caseload.

Person specification


Qualifications

* Registered Nurse.
* Mentor/Preceptor/998 or equivalent.
* Specialist Nursing Practice (District Nursing) Degree or willingness to undertake training.
* Nurse Prescribing or willingness to undertake training.
* Knowledge of recent NHS developments.
* Knowledge of evidence‑based practice.
* Evidence of personal development.
* Knowledge of NMC code of professional conduct.


Experience

* Evidence of experience at Band 5 Community Nurse level.
* Management of a team.
* Management of change within a complex environment.
* Experience of teaching others.
* Proven clinical skill at Senior Community Nursing level.
* Experience of participating in investigation / management of complaints.
* Evidence of participating in clinical audits in the workplace.


Skills

* Communicating within a multi‑disciplinary team.
* Ability to communicate where there may be barriers to understanding.
* Excellent oral and written communication skills.
* Effective interpersonal skills.
* Management of change.
* Negotiation skills.
* Understanding of current political and policy context in Primary Health Care.
* Knowledge of clinical governance and its implications.
* Evidence of expertise in clinical skills.
* Knowledge of research processes.


Other requirements

* Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Staff recruited from outside the NHS will usually be appointed at the bottom of the pay band. All new appointments (excluding Medical & Dental roles) are subject to a 26‑week probation period.
