Responsibilities:
* To support and enable young people to maintain skills and personal interests whilst delivering person-centred care unique to each individual
* To keep training and skill levels up to date and relevant
* Support Young people to maintain their relationships and connections with the local community
* Ensure Care Support Plans and Risk Assessments and other information about how to support Young people are followed
* Be responsible for promoting and safeguarding the welfare of those individuals they support
* This role does not include weekends unless on call
* Work as part of the care team
* Respecting the dignity of Young people and promoting independence.
Requirements:
* Experience in working with young people
* Level 3 Children's and Young People or equivalent (Preferred but not essential)
* Driver
